RAID 5 status fails (Intel RST / Win10)

I have a RAID5 with 3 x 3TB SATA-HDD under Win 10 64bit. Recently one of the disks became unreliable.

The Intel RST Option ROM originally showed ID(3) hard disk Error Occurred

Fix the error after starting Ctrl+I to enter, but the RAID status is still failed

I plug in the new disk(Toshiba DT01ACA3) and set Spare disk still cannot rebuild RAID5

. Is there any way to fix this problem?
